We know that parents often find filling in the Medical / Activity Permission Form a bit of a chore. It is however important that we have an up to date form for each camp.

To make parents lives that little bit easier, we have produced a form that can be filled in on your computer. It can be electronically signed and then either be printed off and handed to your child’s Leader or emailed. A copy can be saved on your computer for future camps.
